NHS Midlands & Lancashire (MLCSU) on behalf of the Commissioner, NHS Staffordshire and Stoke-on-Trent Integrated Care Board (SSOT ICB), is intending to follow the Most Suitable Provider process (MSP) in accordance with Regulation 10 of the Health Care Services (Provider Selection Regime) (PSR) 2023 for its provision of All Age Continuing Care (AACC) Service, with the CPV codes 85100000-0 (Healthcare Services) and 85323000-9 (Community health services, but only in respect of community health services which are delivered to individuals).

This is a contract for the provision of Primary Medical Care Services at the Withnell Health Centre, Chorley. The contract will commence on 01 October 2024 and have an initial term of ten years. The contract may be extended to a maximum of term of fifteen years.

The service is a face-to-face option from 111; It is however only by exception. This means that it is for those patients who are:

- unable to attend a face-to-face appointment e.g. end of life (EOL),

- require all care in bed, house bound and shielding as per Public Health England guidelines or

- who have been assessed by a clinician as appropriate for the service following an initial CAT 3 / 4 ambulance disposition.
